OUTREACH Physical and Occupational Therapy and Speech Rehabilitation provides one-to-one in-home rehabilitation to adults who have difficulty getting to an outpatient clinic or opt to receive care in the privacy and comfort of their own home.
Physical Therapist Alex Greenspan used to work at a home care agency and an outpatient clinic. At the home care agency setting, he continually saw patients getting discharged shortly after as soon as they were medically stable or no longer home bound. When they were referred to an outpatient clinic they would either not go or stop going shortly after stating a number of factors that deterred them such as transportation issues, long waits, being double booked and inclement weather.
This prompted him to start Outreach Rehabilitation in 2004.
“The home care environment is an extremely effective place to work on patients’ functional deficits and needs. It allows a therapist to work in a private, non-distracting setting that is individualized to the patient allowing for a complete picture of what specific goals need to be focused on.”
Outreach now has more than 30 PT, OT and speech therapists who travel to patients’ homes in the New York City area. They see all types of diagnoses with a specific focus on a client’s functional deficits — from relieving pain and strengthening weaknesses, to helping patients comfortably move around their homes. Seemingly basic movements, everyday tasks and activities one would like to participate in can become more difficult following an injury or steady decline in loss of motion, strength or balance. Outreach therapists deal with patients with compassion and patience while trying to keep sessions engaging, effective and productive. They have a special focus on seniors, but also work with younger adults rehabbing from injuries or surgeries as well.
Outreach accepts Medicare along with a few other insurances. A therapist (Physical, Occupational or Speech) can make an initial evaluation within 24-48 hours after the administrative office receives a physician’s prescription.

Hello,
I just found you tonight & I’m writing with a dilemma. To introduce myself: Resident of the Upper West Side, Age 83 (84 next month. Need physical therapy for a number of reasons…primarily mobility related, & persistent pain in upper back. My physician is prescribing a referral for PT with the VNS. How does your service compare with the VNS? Please comment. Thanks.

Hi Mr. Sultan,
Pls. give me a call at 212-842-0099. Will be in office periodically throughout the day on Thursday and Friday. Outreach solely focuses on Rehabilitation. We don’t provide nursing or home attendants. In addition, we are more of a long term Therapy Grp. that bills through Medicare part B insurance whereas VNS is short term and bills through Medicare part A typically. Sincerely, Alex Greenspan
